ANGIO-SONOGRAPHIC PARALLELS OF DIAGNOSIS OF ARTERIAL SYSTEM LESIONS IN PATIENTS WITH PURULENT-NECROTIC COMPLICATIONS OF DIABETIC FOOT SYNDROME

dc.descriptionSummary. Ultrasound Doppler and angiography remain the most effective methods for diagnosing multilevel disease of the arterial system of the lower extremity. The aim of the study – to compare the effectiveness of methods for diagnosing arterial insufficiency in patients with ischemic form of diabetic foot syndrome. Materials and Methods. An analysis of the use of ultrasound Doppler and subtraction digital angiography in 51 patients who were hospitalized in the surgical in-patient unit of the Volyn Regional Hospital for War Veterans in the period from 2015 to 2019 was performed, with purulent-necrotic complications of diabetic foot syndrome and signs of arterial insufficiency. The Cohen’s kappa coefficient was used to compare the effectiveness of each method. Results. According to the results of comparative analysis of the use of Doppler and angiography according to the Cohen’s kappa coefficient, the greatest coincidence was observed in the aorto-iliac and iliac-femoral segments, while the discrepancy was found in the diagnosis of distal segments, especially in obese patients and severe inflammatory changes. Conclusions. Both methods of visualization of lesions of the arterial system are effective with a slight advantage of angiography in the diagnosis of lesions of the distal parts of the arterial system by the Cohen’s kappa coefficient.en-US
dc.descriptionРезюме. УЗ-доплерографія та ангіографія залишаються найефективними методами діагностики багаторівневого захворювання артеріальної системи нижньої кінцівки. Мета дослідження – порівняти ефективність методів діагностики артеріальної недостатності у пацієнтів з ішемічною формою синдрому діабетичної стопи. Матеріали і методи. Проведений аналіз використання УЗ-доплерографії та субтракційної цифрової ангіографії у 51 пацієнта, якого госпіталізували у хірургічний стаціонар Волинського обласного госпіталю ветеранів війни в період із 2015 до 2019 р. з гнійно-некротичними ускладненнями синдрому діабетичної стопи та з ознаками артеріальної недостатності. Для порівняльної оцінки ефективності кожного методу використано коефіцієнт Каппа – Коена. Результати. За результатами порівняльного аналізу використання доплерометрії та ангіографії згідно з показниками коефіцієнта Каппа – Коена найбільше співпадання відмічено в аорто-клубовому та клубово-стегновому сегментах, в той час як розбіжність результатів встановлена в діагностиці дистальних сегментів, особливо пацієнтів з ожирінням та вираженими запальними змінами нижньої кінцівки. Висновки. Обидва методи візуалізації ураження артеріальної системи є ефективними з незначною перевагою ангіографії у діагностиці ураження дистальних відділів артеріального русла за результатами коефіцієнта Каппа – Коена.uk-UA
