РЕЗУЛЬТАТИ ЛІКУВАННЯ УСКЛАДНЕНЬ СИНДРОМУ ДІАБЕТИЧНОЇ СТОПИ З ВИКОРИСТАННЯМ БІОПОВ’ЯЗОК

dc.descriptionSummary. In 2017, the Center for Medical Statistics of the Ministry of Health of Ukraine recorded 2,757.7 million people with diabetes. However, due to latent forms of diabetes, the number of patients in Ukraine is 2–3 times higher than abovementioned. The wound  process in patients with diabetic foot syndrome has its own peculiarities, so it requires the use of special  dressings. The aim of the study – to improve the results of treatment of complications of diabetic foot syndrome. Materials and Methods. Analysis of the results of treatment of 29 patients with complicated forms of diabetic foot by conventional methods and 34 patients who underwent the use of biobandage. The control of the wound process was carried out by examining smears for the presence of the following elements: the number of leukocytes, the characteristics of phagocytosis, as well as other cellular elements of blood and connective tissue. Results. In smears-imprints of both groups of patients the percentage of destruction of leukocytes decreased in time. On the 4th day, in the main group the indicator was 90.19 %, in the control group – 95.65 %; on the 8th day – 20.17 % and 33.89 %; on the 10th day – 1.5 % and 5.32 %, respectively. A significant decrease in the number of incomplete and degenerative types of phagocytosis in relation to the completed in the main group compared with the control was obtained on the 4th and 14th day of the wound process. Bacteriological examination yielded positive culture results and isolated 25 bacterial cultures. The rate of epithelialization in the main group was 4.4–4.6 % per day, compared with the control group, where it was 1.7–2.2 % per day. Conclusions. Comparative evaluation of the results showed the effectiveness of the use of cultured tissue equivalents based on allofibroblasts, in the local treatment of complications of diabetic foot in comparison with traditional drugs.en-US
dc.descriptionРезюме. Центр медичної статистики МОЗ України в 2017 р. зафіксував 2757,7 млн осіб із цукровим діабетом. Однак у зв’язку з прихованими формами кількість хворих в Україні у 2–3 рази вища від вищезазначеної. Рановий процес у хворих із синдромом діабетичної стопи має свої особливості перебігу, тому вимагає застосування спеціальних перев’язувальних матеріалів. Мета дослідження – поліпшити результати хірургічного лікування хворих з ускладненими формами діабетичної стопи, зменшити час і покращити результати загоєння ран після оперативних втручань. Матеріали і методи. Проведено аналіз результатів лікування 29 пацієнтів з ускладненими формами стопи діабетика за загальноприйнятою методикою і 34 хворих, яким застосовували біопов’язки. Контроль ранового процесу здійснювали дослідженням мазків на наявність таких елементів: кількість лейкоцитів, характеристика фагоцитозу, а також інші клітинні елементи крові й сполучної тканини. Результати. У мазках-відбитках обох груп хворих зменшувався в часі відсоток деструкції лейкоцитів. На 4-ту добу в основній групі показник становив 90,19 %, в контрольній групі – 95,65 %; на 8-му – 20,17 та 33,89 %; на 10-ту добу – 1,5 та 5,32 % відповідно. Достовірне зниження кількості незавершеного і дегенеративного видів фагоцитозу відносно завершеного в основній групі, порівняно з контрольною, отримано на 4-ту і 14-ту доби ранового процесу. При бактеріологічному дослідженні були отримані позитивні результати посіву і виділено 25 культур бактерій. Швидкість епітелізації в основній групі дорівнювала 4,4–4,6 % на добу, порівняно з контрольною групою, де вона складала 1,7–2,2 % на добу. Висновки. Порівняльна оцінка результатів показала ефективність застосування культивованих тканинних еквівалентів на основі алофібробластів у місцевому лікуванні ускладнень стопи діабетика порівняно з традиційними препаратами.uk-UA
