Severe Meningococcal Infection Associated with COVID-19 in a Child: a Clinical Challenge in Pediatric Practice

| dc.description | SUMMARY. Meningococcal infection remains one of the most severe and life-threatening bacterial infections in childhood. The COVID-19 pandemic caused by SARS-CoV-2 has significantly altered the epidemiology of infectious diseases and drawn attention to the issue of co-infections. The concomitant course of SARS-CoV-2 infection with other infectious diseases may substantially modify the clinical presentation, complicate timely diagnosis, and adversely affect disease progression and outcomes. This article presents a case of severe generalized meningococcal infection in a previously healthy young child. A distinctive feature of this case was the atypical recurrent course of the disease, characterized by severe meningococcemia progressing to septic shock, purulent meningitis, pneumonia, and arthritis. Such multisystem involvement within a single episode of infection is relatively uncommon and reflects both the exceptionally invasive nature of the pathogen and the pronounced systemic inflammatory response. The presented case highlights the potential impact of SARS-CoV-2 co-infection on the clinical course of invasive meningococcal disease and emphasizes the importance of maintaining a high level of clinical vigilance for severe bacterial complications in children with COVID-19. An additional clinical challenge in this case was the laboratory-confirmed SARS-CoV-2 co-infection, which may have altered the host immune response and contributed to the severity and clinical dynamics of the bacterial infection. Conclusions. The coexistence of SARS-CoV-2 and Neisseria meningitidis may contribute to the development of a complex pathogenic cascade in which virus-induced immune-inflammatory and vascular disturbances potentially facilitate the invasive course of bacterial infection. Early identification of N. meningitidis and prompt initiation of comprehensive intensive care are crucial determinants of a favorable clinical outcome and complete recovery. This case underscores the importance of maintaining a high index of suspicion for invasive bacterial infections in children with COVID-19, particularly in the presence of rapid clinical deterioration or signs of systemic inflammatory response. | en-US |
| dc.description | Менінгококова інфекція залишається однією з найбільш небезпечних бактерійних інфекцій у дитячому віці. Пандемія COVID-19, спричинена вірусом SARS-CoV-2, суттєво змінила структуру інфекційної захворюваності та привернула увагу до проблеми ко-інфекцій. Поєднаний перебіг SARS-CoV-2-інфекції з іншими інфекційними захворюваннями може суттєво модифікувати клінічну картину, ускладнювати своєчасну діагностику та негативно впливати на перебіг і прогноз хвороби. У статті наведений випадок тяжкої генералізованої менінгококової інфекції у раніше практично здорової дитини раннього віку. Особливістю випадку є атиповий рецидивний перебіг захворювання, що включає поєднання тяжкої менінгококцемії з прогресуванням до септичного шоку, гнійного менінгіту, пневмонії, артриту. Така мультисистемність ураження в межах одного епізоду інфекції є відносно рідкісною та свідчить про надзвичайно високу інвазивність збудника і потужну системну запальну відповідь. Додаткову клінічну складність становить ко-інфекція SARS-CoV-2, підтверджена лабораторно, що могла модифікувати імунну відповідь організму та вплинути на тяжкість і динаміку бактерійної інфекції. Висновки. Поєднання SARS-CoV-2 та Neisseria meningitidis може сприяти формуванню складного патогенетичного каскаду, в якому вірус-індуковані імунозапальні та судинні порушення потенційно створюють умови для інвазивного перебігу бактерійної інфекції Рання ідентифікація N. meningitidis і комплексна інтенсивна терапія є ключовими чинниками сприятливого клінічного висліду та одужання дитини. | uk-UA |
