MEDICATION ADHERENCE IN OUTPATIENTS WITH HEART FAILURE
ПРИХИЛЬНІСТЬ ДО ЛІКУВАННЯ У ПАЦІЄНТІВ ІЗ СЕРЦЕВОЮ НЕДОСТАТНІСТЮ В АМБУЛАТОРНИХ УМОВАХ

| dc.description | Introduction. In the management of heart failure, adherence to prescribed pharmacological therapy and maintaining a high level of treatment compliance are key factors. The aim of this study was to assess the impact of the "Heart Failure Symptom Monitoring Diary" on treatment adherence among outpatients with heart failure. Main part. The study included 55 patients from the "City Polyclinic No. 2" in Chernivtsi. In the cardiology office, they received individual counseling, education, and a diary for daily self-monitoring of heart failure symptoms in outpatient settings. It was found that educational interventions improved adherence to pharmacotherapy from 39.2 % to 71.2 %, and adherence to a healthy lifestyle from 31.5 % to 82 %. Forgetfulness, missed doses, self-discontinuation of medications, and neglect of medication records significantly decreased (by 67.3 %, 58.2 %, 45.5 %, and 67.2 %, respectively) (p = 0.0007). A high level of adherence (9.1 points) after the intervention was achieved in 71 % of patients, moderate adherence in 21.8 %, and low adherence in only 7.2 % (p = 0.020). The average adherence score increased to 8.1 compared to 4.4, indicating a significant improvement. The vast majority of patients (92.7 %, n = 51) demonstrated excellent adherence (≥8 points) after the educational intervention. Among elderly female patients, who predominated in the study group, adherence increased from moderate to high (8.2 points vs. 5.1). Self-monitoring indicators improved, in particular: the frequency of regular weight monitoring increased by 62%, fluid intake control (within the recommended volume of 1–1.5 L per day) improved by 40.2%, and adherence to dietary recommendations regarding sodium restriction increased by 46.2%. Conclusions. Targeted education and the use of a monitoring diary are effective educational tools for patients with heart failure, promoting greater disease awareness and treatment adherence, improving self-monitoring, facilitating lifestyle changes in outpatient settings, and supporting the implementation of individualized treatment plans. | en-US |
| dc.description | Вступ. У менеджменті серцевої недостатності ключовим чинником є дотримання призначеної медикаментозної терапії та забезпечення високого рівня прихильності до лікування. Метою роботи є оцінка впливу «Щоденника моніторингу симптомів серцевої недостатності» на прихильність до лікування у пацієнтів із серцевою недостатністю в амбулаторних умовах. Основна частина. В дослідженні взяли участь 55 пацієнтів «Міської поліклініки №2» м. Чернівці. В кабінеті кардіолога вони отримали індивідуальне консультування, навчання та «щоденник» для щоденного самоконтролю за симптомами серцевої недостатності в амбулаторних умовах. Встановлено, що освітні втручання підвищили прихильність до фармакотерапії з 39,2% до 71,2%, а до здорового способу життя з 31,5% до 82%. Забудькуватість, пропуски доз, самовільна відміна препаратів і нехтування записами про прийом ліків, суттєво зменшились (відповідно на 67,3%, 58,2%, 45,5% та 67,2%) (p=0,0007). Високий рівень прихильності (9,1 балів) після втручання досягнуто у 71% хворих, середній — у 21,8%, низький — лише у 7,2% (p = 0,020). Середній бал зріс до 8,1 (проти 4,4), у що свідчить про високу прихильність. Абсолютна більшість пацієнтів (92,7%, n = 51) після освітніх втручань мали відмінну прихильність до лікування (8 балів). У жінок старечого віку, які переважали у нашому дослідженні, рівень прихильності підвищився з середнього до високого (8,2 балів проти 5,1). Покращились показники самоконтролю, зокрема, частота регулярного зважування зросла на 62 %, контроль споживання рідини (в межах рекомендованого обсягу до 1–1,5 л на добу) — на 40,2 %, а дотримання дієтичних рекомендацій щодо обмеження кухонної солі — на 46,2 %. Висновки. Проведення цілеспрямованого навчання та ведення «щоденника» є ефективним освітнім інструментом у пацієнтів із серцевою недостатністю, яке сприяє підвищенню їх обізнаності щодо хвороби та прихильності до лікування, покращенню самоконтролю, зміни способу життя в амбулаторних умовах та виконання індивідуального плану лікування. | uk-UA |
