dc.descriptionThe problem of dental health in the pediatric population is highly significant. Malocclusion and tooth position along with periodontal tissue diseases occupy leading positions in the structure of morbidity. Orthodontic treatment often becomes additional causative factor provoking inflammatory processes in the periodontium. The aim of the study. To analyze scientific literature regarding the etiopathogenetic mechanisms of inflammatory periodontal disease development in children with malocclusion and and tooth position during orthodontic treatment stages, and to evaluate the efficacy of modern treatment and prevention methods. Materials and Methods. A literature review of scientific publications was conducted using the systems “PubMed”, “Google Scholar”, and the V. I. Vernadsky National Library for 2018–2025. The search was conducted using the following keywords and their combinations: “orthodontic treatment”, “periodontal disease in children”, “gingivitis”, “dental anomalies”, “biofilm”, “oral dysbiosis”, “probiotics in dentistry”. Results. It was established that malocclusion and orthodontic appliances act as combined risk factors. It has been proven that fixed appliances contribute to the colonization of periodontal pathogens (in particular, the “red complex”) due to additional retention points, reduced resistance, and the development of oral dysbiosis. The mechanisms of inflammation are reviewed, from immunological response (cytokine imbalance) to direct mechanical injury (functional overload). The effectiveness of preventive measures, including herbal remedies, probiotics, and minimally invasive hygiene methods, is analyzed. Conclusions. It has been proven that the key triggering mechanism for the occurrence of inflammatory periodontal diseases in children during orthodontic treatment is biofilm. The influence of pathogenic microflora, mechanical trauma stimulate the synthesis of pro-inflammatory cytokines, activation of osteoclastogenesis, MMP. It has been established that additional inflammatory factors in the form of anatomical and morphological features, iatrogenic factors, specific design and composition of orthodontic appliances stimulate and intensify inflammation of periodontal tissues. The effectiveness of safer alternatives to chlorhexidine, in particular biocorrection and phytotherapy, and the need for a comprehensive approach, systemic support, and repeated hygiene education of children are noted.en-US
dc.descriptionПроблема стоматологічного здоров’я дитячого населення є однією з найактуальніших. Аномалії прикусу та положення зубів разом із захворюваннями тканин пародонта посідають провідні місця у структурі захворюваності. Ортодонтичне лікування часто стає додатковим причинним фактором, що провокує запальні процеси в пародонті. Мета дослідження – провести аналіз наукової літератури щодо етіопатогенетичних механізмів розвитку запальних захворювань пародонта у дітей з аномаліями прикусу та положення зубів на етапах ортодонтичного лікування, а також оцінити ефективність сучасних методів лікування та профілактики. Матеріали та методи дослідження. Було проведено літературний огляд наукових публікацій за допомогою систем PubMed, Google Scholar, Національної бібліотеки імені В. І. Вернадського за 2018–2025 рр. Пошук здійснювався за такими ключовими словами та їх комбінаціями: «ортодонтичне лікування», «захворювання пародонта у дітей», «гінгівіт», «зубощелепні аномалії», «біоплівка», «дисбіоз порожнини рота», «пробіотики в стоматології». Результати. Встановлено, що патологія прикусу й ортодонтичні конструкції діють як поєднані фактори ризику. Доведено, що незнімна апаратура сприяє колонізації пародонтопатогенів (зокрема, «червоний комплекс») через додаткові ретенційні пункти, зниження резистентності та розвиток дисбіозу ротової порожнини. Розглянуто механізми запалення, від імунологічної відповіді (цитокіновий дисбаланс) до прямої механічної травми (функціональне перевантаження). Проаналізовано ефективність профілактичних засобів, включно з фітопрепаратами, пробіотиками та малоінвазивними методами гігієни. Висновки. Доведено, що ключовим пусковим механізмом виникнення запальних захворювань пародонта у дітей під час ортодонтичного лікування є біоплівка. Вплив патогенної мікрофлори, механічна травма стимулюють синтез прозапальних цитокінів, остеокластів, MMP. Встановлено, що додаткові фактори запалення у вигляді анатомо-морфологічних особливостей, ятрогенних чинників, специфіки конструкції та складу ортодонтичних апаратів стимулюють і посилюють запалення тканин пародонта. Зазначено ефективність більш безпечних альтернатив хлоргексидину, зокрема біокорекція та фітотерапія та необхідність комплексного підходу, системної підтримки та повторного гігієнічного навчання дітей.uk-UA
