ВПЛИВ ЛЕЙОМІОМИ МАТКИ НА УСКЛАДНЕННЯ ПЕРШОГО ТРИМЕСТРУ ВАГІТНОСТІ

dc.descriptionThe aim of the study – to study the features of complications of the first trimester of pregnancy in women with uterine leiomyoma (LM) who were not operated on before pregnancy compared with those who underwent conservative myomectomy. Material and Methods. 514 pregnant women were examined, 456 of them with uterine fibroids (1, main group). 351 pregnant women with uterine LM were without surgical treatment before pregnancy (subgroup 1a) and 105 pregnant women were after conservative myomectomy (59 patients were after laparoscopic myomectomy of the subserous node or hysteroscopic – submucous node with coagulation of the bed (1b subgroup), 46 pregnant women after laparotomy myomectomy (1v subgroup)). Comparison group (Group 2) consisted of 82 pregnant women without uterine LM with pregnancy complications. Control (group 3) included 81 pregnant women without uterine LM and without pregnancy complications. Results and Discussion. The conducted studies indicate that the threat of termination of pregnancy in the early stages, retrochorial hematoma, low placentation was more common in women with uterine fibroids than in the group that underwent conservative myomectomy before pregnancy. Depending on the treatment of LM, vaginitis was more common in pregnant women of the 1a subgroup – in almost every third patient who needed sanitation. In pregnant women of subgroups 1b and 1v – almost every fifth. Perhaps, against the background of an infectious factor in pregnant women of the main group, there was a threat of termination of pregnancy. Conclusions. The conducted studies indicate the need for conservative myomectomy in women with uterine fibroids at the pregravid stage, which reduces the complication of the course of the first trimester of pregnancy.en-US
dc.descriptionМета дослідження – вивчити особливості ускладнень першого триместру вагітності у жінок із лейоміомою матки, які не оперувались до вагітності порівняно з тими, яким була проведена консервативна міомектомія. Матеріали та методи. Обстежено 514 вагітних жінок, із них 456 мали лейоміому (перша, основна група). 351 вагітна з лейоміомою були без хірургічного лікування до вагітності (1а підгрупа) та 105 вагітних були після консервативної міомектомії (59 пацієнток було після лапароскопічної міомектомії субсерозного вузла або гістероскопічної – субмукозного вузла з коагуляцією ложа (1б підгрупа), 46 вагітних – після лапаротомічної міомектомії (1в підгрупа)). Групу порівняння склали 82 вагітні жінки без лейоміоми з ускладненнями вагітності. До контрольної, 3 групи, увійшла 81 вагітна жінка без лейоміоми та без ускладнень вагітності. Результати досліджень та їх обговорення. Проведені дослідження свідчать про те, що загроза переривання вагітності в ранні терміни, ретрохоріальна гематома, низька плацентація частіше зустрічались у жінок з лейоміомою, ніж в групі, яким була проведена консервативна міомектомія до вагітності. Залежно від проведеного лікування лейоміоми вагініт частіше зустрічався у вагітних 1а підгрупи – майже у кожної третьої пацієнтки, який потребував санації. У вагітних 1б та 1в підгрупи – майже у кожної п’ятої. Можливо, на фоні інфекційного фактора у вагітних основної групи виникала загроза переривання вагітності. Висновки. Проведені дослідження свідчать про необхідність консервативної міомектомії у жінок із лейоміомою матки на прегравідарному етапі, що дає можливість зменшити ускладнення перебігу першого триместру вагітності.uk-UA
