| dc.description | The aim of the study is to highlight the features of obstetric complications course in pregnant women with chronic obstructive pulmonary disease depending on the functional condition of the respiratory system. Materials and Methods. 47 pregnant women with chronic obstructive pulmonary disease were examined. Pregnant women from the main group were divided into IA (23 women with mild bronchoobstruction) and IB (24 women with moderate bronchoobstruction) subgroups. 28 healthy pregnant women were consisted in the control group. A comparative analysis of obstetric complications were used and changes in the functional state of the respiratory system which play a role in the development of this patient. Results. The examination of the main group of pregnant women with chronic obstructive pulmonary disease was showed that a characteristic symptoms of chronic obstructive pulmonary disease have a pronounced tendency to progress in the second half of pregnancy: chronic cough (34.8 % in IA and 62.5 % in IB subgroup), shortness of breath (34.8 % and 41.7 % in accordance), sputum production (26.1 % and 37.5 % in accordance), chest tightness (21.7 % and 33.3 % in accordance) (р≤0,05). A higher frequency of obstetric and perinatal complications was noted in patients with a decreasing in external respiration rates. Placental dysfunction (56.5 % in IA and 66.7 % in IB subgroups), threatened of preterm birth (34.8 % and 54.2 % in accordance), intrauterine growth restriction (13.0 % and 16.7 % in accordance), low birth weight (30.4 % and 41.7 % in accordance) (p≤0.05). Conclusions. This paper has clearly shown that a deterioration in respiratory function became the reason of increasing the obstetric and perinatal complications frequency in pregnant women with chronic obstructive pulmonary disease in the second half of gestation. It requires, for the purpose of prevention obstetric pathology, to develop the modern approaches that help to manage such patients. | en-US |
| dc.description | Мета дослідження – проаналізувати особливості перебігу акушерських ускладнень в залежності від функціонального стану дихальної системи у вагітних з хронічним обструктивним захворюванням легень. Матеріали та методи. Обстежено 47 вагітних жінок із ХОЗЛ. Вагітні з основної групи залежно від ступеня бронхообструкції були поділені на ІА (23 жінки з легким ступенем бронхообструкції) та ІБ (24 жінки з помірним ступенем бронхообструкції) підгрупи. Контрольну групу склали 28 практично здорових вагітних жінок. Проведено порівняльний аналіз акушерських ускладнень, встановлено зміни функціональнго стану дихальної системи, що відіграють роль у розвитку цих ускладнень. Результати. При обстеженні вагітних із ХОЗЛ основної групи спостерігалася виражена тенденція до прогресування у них в другій половині вагітності симптомокомплексу, який характерний для ХОЗЛ: хронічний кашель (34,8 % у ІА та 62,5 % у ІБ підгрупі), задишка (34,8 % та 41,7 % відповідно), виділення мокротиння (26,1 % та 37,5 % відповідно), відчуття стиснення в грудях (21,7 % та 33,3 % відповідно) (р0,05). Зі зниженням показників зовнішнього дихання відмічали вищу частоту розвитку акушерських та перинатальних ускладнень у цих пацієнток. Найчастіше мале місце дисфункція плаценти (56,5 % у ІА та 66,7 % у ІБ підгрупі), загроза передчасних пологів (у 34,8 % та 54,2 % відповідно), СЗРП (у 13,0 % та 16,7 % відповідно), мала вага дітей при народженні (30,4 % та 41,7 % відповідно) (р≤0,05). Висновки. У вагітних жінок з ХОЗЛ в другій половині гестації спостерігається погіршення функції зовнішнього дихання, яке супроводжується збільшенням частоти акушерських та перинатальних ускладнень, що потребує розробки сучасних підходів до ведення таких пацієнток з метою попередження акушерської патології. | uk-UA |
