ПРОБЛЕМА ПНЕВМОКОКОВИХ МЕНІНГІТІВ ЯК НАЙБІЛЬШ НЕБЕЗПЕЧНОЇ ІНВАЗИВНОЇ ФОРМИ ПНЕВМОКОКОВИХ ІНФЕКЦІЙ

dc.descriptionSUMMARY. The development of vaccines against pneumococcal infection and the introduction of pneumococcal conjugate vaccines (PCVs) into national immunization schedules in many countries worldwide in the early 2000s were aimed at reducing the incidence of invasive forms of this infection in children, including pneumococcal meningitis (PM). The aim of the study was to analyze the epidemiological features of PM globally, including under the influence of widespread PCV use, and in Ukraine (2010–2024), as well as to assess the prospects for its further prevention. The study demonstrates differences in incidence rates (by tens of times) and in the etiological serotype landscape of pneumococci across different countries. The effectiveness of PCVs against PM caused by pneumococcal serotypes included in vaccines (VST) is shown, to a greater extent in regions with a temperate climate. When selecting strategies for routine childhood immunization, the evolutionary potential of pneumococci was not taken into account, particularly regarding the replacement of epidemiologically relevant serotypes under vaccine pressure by non-vaccine serotypes, the acquisition of invasive properties by non-encapsulated pneumococci, and the problem of antibiotic resistance. As a result, the effectiveness of such vaccination continues to decline, prompting the introduction of PCVs with higher valency as well as different vaccine compositions, which leaves the problem of pneumococcal infection still relevant today, particularly in connection with increasing incidence among individuals aged >50–65 years. This necessitates a revision of vaccination strategies. In Ukraine, PCVs are not included in the age-based national immunization schedule; however, the incidence of PM among both children and the general population (0.01–0.23 per 100,000 population) is lower than in countries where vaccination has been implemented for many years. Among children under 5 years of age, it ranged from 1.3 (2011) to 0.13 per 100,000 children of this age group (2024), with a downward trend; in 2021–2022, no cases were registered. Among children under 1 year of age, it fluctuated from 1.57 (2010) and 2.4 (2011, 2014) to 1.19 (2019); in 2020 it was 0.32, and in 2021–2024 no cases were reported. However, pneumococcal serotyping is not conducted on a routine basis. Based on PM incidence estimates in at-risk age groups, the introduction of routine PCV immunization in Ukraine appears impractical. A rational approach under these conditions is vaccination of medical risk groups and vaccination according to epidemic indications (including military personnel) to ensure more effective protection against VST using vaccines with the highest possible valency. This approach may contribute to slowing the evolution of pneumococci in Ukraine toward increased invasiveness of non-vaccine serotypes and non-encapsulated variants.en-US
dc.descriptionРозробка вакцин проти пневмококової інфекції та впровадження на початку 2000-х років пневмококових кон’югованих вакцин (PCVs) в Календар щеплень у багатьох країнах світу були націлені за зменшення захворюваності на інвазивні форми цієї інфекції у дітей, в тому числі й пневмококового менінгіту (РМ). Метою роботи було проаналізувати епідеміологічні особливості РМ у світі, зокрема і під впливом широкого застосування PCVs, та в Україні (2010–2024 рр.), а також оцінити перспективу їх подальшої профілактики. У роботі показано відмінності в захворюваності (у десятки разів), етіологічному серопейзажі пневмококів у різних країнах. Показано ефективність PCVs щодо РМ, зумовлених пневмококами тих серотипів, що входять до складу вакцин (VST), більшою мірою для регіонів із помірним кліматом. При виборі стратегії планової вакцинації дітей не було враховано еволюційний потенціал пневмококів щодо заміни епідемічно актуальних серотипів під тиском вакцинації на невакцинні серотипи, набуття некапсульованими пневмококами інвазивних властивостей, проблеми антибіотикорезистентності. За рахунок цього ефективність такої вакцинації продовжує знижуватися, що спонукає до впровадження PCVs як із більшою валентністю, так і за різним складом вакцин, що залишає проблему пневмококової інфекції актуальною на сьогодні, зокрема у зв’язку з підвищенням захворюваності у людей віком >50–65 років. Це потребує перегляду стратегії вакцинації. В Україні PCVs не входять до Календаря щеплень «за віком», однак захворюваність на РМ як серед дітей, так і населення загалом (0,01–0,23 на 100 000 населення), є нижчою, ніж у країнах, де вакцинація застосовується багато років. Серед дітей <5 років вона становила від 1,3 (2011) до 0,13 на 100 000 дітей цього віку (2024) із тенденцією до зниження, у 2021–2022 рр. не зареєстрованого жодного випадку. Серед дітей <1 року вона коливалася від 1,57 (2010), 2,4 (2011, 2014) до 1,19 (2019), у 2020 р. дорівнювала 0,32, а в 2021–2024 рр. не реєструвалася. Однак серотипування пневмококів на постійній основі не здійснюється. Введення планової вакцинації PCVs в Україні за оцінками захворюваності на РМ у вікових групах ризику недоцільне. Раціональним підходом за такої ситуації є щеплення медичних груп ризику та за епідемічними показаннями (зокрема і військовиків) для їх ефективнішого захисту від VST вакцинами з максимальною валентністю, що сприятиме уповільненню процесу еволюції пневмококів в Україні щодо набуття інвазивності невакцинними серотипами та некапсульованими варіантами.uk-UA
