dc.descriptionSUMMARY. Exosomes are extracellular nanostructures with a diameter of 30–150 nm that are secreted by various cell types and play a crucial role in intercellular communication and tissue homeostasis. Due to their ability to transport bioactive molecules – microRNAs, proteins, lipids, DNA fragments, and metabolites – exosomes act as key regulators of the cellular microenvironment, angiogenesis, proliferation, and tissue repair. Exosomes derived from mesenchymal stem cells (Hanheal, PlatinumMed) possess a unique biomolecular cargo profile and demonstrate a high regenerative potential in clinical and aesthetic medicine. In dermatology, exosomes activate dermal fibroblasts, keratinocytes, and melanocytes, enhance the synthesis of extracellular matrix components, stimulate dermal remodeling, improve skin barrier function, and reduce manifestations of photoaging and chronologic aging. These properties define exosomes as a promising biological tool for the development of personalized regenerative protocols in dermatology and aesthetic medicine. The aim – to evaluate the clinical efficacy of Hanheal-derived exosomes and their combination with polynucleotides (PDRN) in improving skin condition using instrumental and subjective assessment methods. Material and Methods. The study involved 63 patients aged 30–60 years, divided into two groups: 31 received exosomal therapy (Hanheal), and 32 underwent a combined protocol (exosomes + PDRN). Skin condition was assessed by objective methods (ultrasound skin examination, skin fold measurement, rotational–compression test) and subjective scales (Likert scale, GAIS, Dermatology Life Quality Index – DLQI). Results. Both treatment protocols resulted in a statistically significant (p<0.001) improvement in the morphofunctional condition of the skin, a decrease in DLQI scores, and higher ratings on the Likert and GAIS scales. Ultrasound data confirmed stimulation of neocollagenesis and pronounced dermal remodeling. The findings demonstrate not only objective improvement of the skin but also a positive impact on patients’ psycho-emotional well-being, which contributes to overall quality of life. Conclusions. Exosomal therapy with Hanheal, both as monotherapy and in combination with PDRN, provides a marked bioregenerative effect, improving skin elasticity, density, and turgor. The results support the feasibility of implementing this method into the clinical practice of cosmetologists, dermatologists, and regenerative medicine specialists. Exosomes can be regarded as a key component in the development of modern personalized strategies for skin rejuvenation, restoration, and maintenance of functional integrity.en-US
dc.descriptionРЕЗЮМЕ. Екзосоми – це позаклітинні наноструктури, які виділяються різними типами клітин і відіграють ключову роль у міжклітинній комунікації та підтриманні гомеостазу тканин. Завдяки здатності транспортувати біоактивні молекули – мікроРНК, білки, ліпіди, фрагменти ДНК та метаболіти – екзосоми є важливими регуляторами клітинного мікрооточення, ангіогенезу, проліферації та репарації. Особливу увагу привертають екзосоми, отримані з мезенхімальних стовбурових клітин (Hanheal, PlatinumMed), які мають унікальний біомолекулярний профіль та демонструють високий потенціал у регенеративній медицині. У дерматології екзосоми сприяють активації дермальних фібробластів, кератиноцитів і меланоцитів, посилюють синтез компонентів позаклітинного матриксу, стимулюють ремоделювання дерми, покращують бар’єрну функцію шкіри й зменшують прояви фото- та хроностаріння. Завдяки цим властивостям вони розглядаються як перспективний біологічний інструмент для розробки персоналізованих регенеративних протоколів у дерматології та естетичній медицині. Мета дослідження – оцінити клінічну ефективність екзосом Hanheal та їх поєднання з полінуклеотидами (ПДРН) у покращенні стану шкіри з використанням апаратних та суб’єктивних методик оцінки. Матеріал і методи. У дослідженні взяли участь 63 пацієнти віком 30–60 років. Пацієнтів поділили на дві групи: 31 особа отримувала екзосомальну терапію Hanheal, 32 – комбінований протокол екзосоми + ПДРН. Оцінювання проводили за об’єктивними параметрами (ультразвукове дослідження шкіри, вимірювання шкірної складки, ротаційно-компресійний тест) та суб’єктивними шкалами (Лайкерта, GAIS, дерматологічний індекс якості життя – DLQI). Результати. У пацієнтів обох груп спостерігалося достовірне покращення морфофункціонального стану шкіри (p<0,001), зменшення значень DLQI та підвищення оцінок за шкалами Лайкерта та GAIS. Ультразвукове дослідження засвідчило стимуляцію неоколагенезу та виражене ремоделювання дерми. Отримані результати підтверджують як об’єктивне покращення стану шкіри, так і позитивний вплив процедур на психоемоційний стан пацієнтів, що є важливим критерієм якості життя. Висновки. Екзосомальна терапія Hanheal, як у монотерапевтичному варіанті, так і в поєднанні з ПДРН, забезпечує виражений ефект біорегенерації, підвищення еластичності, щільності та тургору шкіри. Результати дослідження свідчать про доцільність широкого впровадження цієї методики у практику лікарів-косметологів, дерматологів і фахівців із регенеративної медицини. Екзосоми можна вважати ключовою ланкою у формуванні сучасних персоналізованих підходів до відновлення, омолодження та підтримання функціональної цілісності шкіри.uk-UA
