АЛЕРГІЯ НА КОРОВ’ЯЧЕ МОЛОКО ЧИ ЙОГО НЕПЕРЕНОСИМІСТЬ У НЕМОВЛЯТ: СУЧАСНИЙ ПОГЛЯД НА АКТУАЛЬНУ ПРОБЛЕМУ

dc.descriptionSUMMARY. Cow’s milk is among the first foods introduced into an infant’s diet and can immediately provoke adverse reactions such as lactose intolerance and cow’s milk allergy. These conditions are often confused or diagnosed late, despite having different pathophysiological mechanisms. The prevalence of lactose intolerance and cow’s milk allergy in the general population is approximately 1–3%, with the highest incidence occurring in infants, posing a significant challenge to their physiological development. The aim – to analyze current scientific literature in order to assist clinicians in the timely diagnosis of lactose intolerance and cow’s milk allergy. Materials and Methods. This review is based on published findings from recent scientific studies addressing the diagnosis of lactose intolerance and cow’s milk allergy. It presents a synthesis of the reviewed sources and the expert opinion of the authors. Results. The gastrointestinal symptoms of lactose intolerance and cow’s milk allergy are often similar, which frequently leads to misinterpretation or delayed diagnosis. For breastfed infants with either condition, breastfeeding should be encouraged, while the maternal diet should exclude dairy products and soy. In cases of lactose intolerance, supplementation with lactase enzyme may be beneficial. If symptoms persist, the use of extensively hydrolyzed formula should be considered. Formula-fed infants with lactose intolerance should be treated with low-lactose or lactose-free formulas, while those with cow’s milk allergy should receive formulas containing extensively hydrolyzed cow’s milk protein or amino acid-based formulas. Conclusions. This review aims to support clinicians in better understanding the mechanisms underlying lactose intolerance and cow’s milk allergy and in distinguishing the overlapping and distinct clinical features of these conditions in early childhood.en-US
dc.descriptionРЕЗЮМЕ. Коров'яче молоко є одним із продуктів, які потраплять в раціон малюка одним із перших та може відразу викликати негативні реакції, такі як непереносимість лактози та алергія на коров'яче молоко. Їх часто плутають або невчасно діагностують, хоча механізм виникнення цих станів є різним. Поширеність непереносимості чи алергії на коров’яче молоко у загальній популяції становить близько 1–3 %, вона є найвищою саме у немовлят і створює проблему для фізіологічного розвитку дітей раннього віку. Мета – аналіз сучасного наукового інформаційного контенту з метою допомогти клініцистам вчасно діагностувати непереносимість лактози і алергію на коров’яче молоко. Матеріал і методи. В роботі використовувалися опубліковані результати сучасних наукових досліджень з проблеми діагностики непереносимості лактози та алергії на коров’яче молоко. Цей огляд є синтезом опрацьованих джерел та експертної думки авторів. Результати. Шлунково-кишкові прояви непереносимості лактози та алергії на коров’яче молоко є досить схожими, тому часто їх неправильно чи невчасно трактують. Матерів немовлят на грудному вигодовуванні з алергією на коров’яче молоко чи його непереносимістю необхідно заохочувати до продовження годування грудьми, з виключенням молочних продуктів та сої з раціону матері, а також з використанням ферменту лактази при його непереносимості. Якщо симптоми не зникають, необхідно розглянути можливість використання екстенсивно гідролізованої суміші. Немовлят на штучному вигодовуванні з непереносимістю лактози слід лікувати сумішшю зі зниженим вмістом лактози або без неї, тоді як немовлят з алергією на коров’яче молоко слід лікувати сумішшю, що містить значно гідролізований білок коров'ячого молока або амінокислоти. Висновки. Наведений аналіз допоможе клініцистам краще розуміти механізми виникнення непереносимості лактози та алергії на коров’яче молоко, а також розпізнавати подібні та відмінні особливості у перебігу даної патології в дітей раннього віку.uk-UA
