ОСОБЛИВОСТІ ПАЦІЄНТ-ОРІЄНТОВАНОГО СПІЛКУВАННЯ ІЗ ПАЦІЄНТОМ З АРТЕРІАЛЬНОЮ ГІПЕРТЕНЗІЄЮ

dc.descriptionSUMMARY. Arterial hypertension (AH) is the cause of 13 % of all deaths in the world. Every year the number of patients with high blood pressure will increase. Despite a large number of pharmacological and non-pharmacological treatments for hypertension, the number of patients with insufficient control of the disease remains high worldwide. The aim – to review the list of the most recent sources of information on the provision of patient-centred medical care for the management of patients with hypertension. Results. Treatment of hypertension is the most effective when the patient cooperates and fully participates in the treatment process, including adherence to the treatment, self-monitoring, and monitoring of blood pressure. A number of studies have found specific effects of communication on self-care and adherence. Another study identified the kind of wording patients want to hear when talking to a doctor about high blood pressure. In addition to the specifics of communication, there are now international guidelines that support treatment methods to improve patient-physician compliance. Several factors need to be considered: identifying drug-related side effects and appropriate dosage levels, using long-acting drugs that require a single dose, avoiding complex dosing regimens, using single pill combinations whenever possible, considering the patient's financial ability to pay for a particular regimen in the long term, if appropriate, or other relevant aspects of local or national health systems, and enlisting the support of a family member or other social support. Conclusions. There is a need to improve the communication skills of doctors and to emphasise the importance of patient awareness of self-care. Only comprehensive measures can ensure a sufficient response to hypertension treatment and help patients live longer and better lives.en-US
dc.descriptionРЕЗЮМЕ. Артеріальна гіпертензія АГ є причиною 13 % усіх смертей у світі. З кожним роком кількість пацієнтів з підвищеним артеріальним тиском буде зростати. Не дивлячись на велику кількість фармакологічних та не фармакологічних методів лікування АГ, кількість пацієнтів з недостатнім контролем захворювання у всьому світі залишається високим. Мета – оглянути перелік найновітніших джерел інформації щодо надання пацієнт-орієнтованої медичної допомоги до ведення пацієнтів з артеріальною гіпертензією. Результати. Лікування АГ є найефективнішим, коли пацієнт співпрацює та повністю бере участь у процесі лікування, що включає дотримання лікування, виконання самоконтролю та моніторинг ознак високого кров’яного тиску. Ряд досліджень виявив особливості впливу спілкування на самообслуговування та прихильність до терапії. Ще одне дослідження виявило, які саме формулювання хоче почути пацієнт при спілкуванні з лікарем щодо питань підвищеного артеріального тиску. Окрім особливостей спілкування на сьогодні є міжнародні регламентуючі документи, які обґрунтовують методи лікування, задля покращення комплаєнсу між пацієнтом та лікарем. Необхідно враховувати кілька факторів: виявлення побічних ефектів, пов’язаних із прийомом препарату, і відповідних рівнів дозування, використання препаратів тривалої дії, які вимагають одноразового дозування, уникнення складних схем дозування, використання комбінацій однієї таблетки, коли це можливо, врахування фінансової спроможності пацієнта заплатити за певний режим у довгостроковій перспективі, якщо це доцільно, або інші відповідні аспекти місцевої або національної систем охорони здоров’я та залучення підтримки члена сім’ї чи іншої соціальної підтримки для поліпшення прихильності до лікування та наполегливості. Висновки. Необхідно покращити комунікативні навички лікарів та наголошувати на важливості обізнаності пацієнта щодо самодопомоги. Тільки комплексні заходи зможуть забезпечити достатню відповідь на лікування АГ та допоможуть пацієнту мати більш тривале та якісне життя.uk-UA
