INFLUENCE OF INTERMITTENT PNEUMATIC COMPRESSION ON PERIPHERAL PARAMETERS IN PERSONS WITH DIFFERENT LEVELS OF BLOOD FILLING THE LOWER EXTREMITIES

| dc.description | SUMMARY. One of the therapeutic methods that has proven its clinical effectiveness as a promising alternative to pharmacological and surgical interventions is intermittent pneumatic compression. The aim – to study of the reaction of peripheral hemodynamics to the effect of intermittent pneumatic compression (IPC) depending on the level of blood filling of the lower extremities. Material and Methods. The group of 62 participants in age of 18–22 years were investigated. Testing session that included pressure of 40 mmHg and 20-minute period of temporary occlusion of the lower extremities was performed by intermittent pneumatic compression with Recovery Pump PRX device. (“Mego Aftek AC Ltd”, Israel). For monitoring hemodynamic changes were applied impedance cardiography (ICG) method: (“ХАІ-medica”, Kharkiv, Ukraine), by 15 sec. Blood filling was calculated according to the formula BF=L2/Z, where BF is the level of blood filling of the lower extremities, L – is the distance between the current electrodes (cm), Z – is the level of the basic impedance (Ohm). According to the values of these indicators, the 1st group with a low level of BF from 2.16 to 5.9, (n=30) and the 2nd group with a high BF from 6.01 to 11.01, (n=32) were determined. Results. The analysis of peripheral hemodynamic indicators between groups revealed that during the recovery period, A1 increased in individuals of the 2nd group, and A2 decreased. In the st group pulse wave velocity was decreased. In addition, index of vascular wall elasticity was incresed in 2nd group. Parameter T was increased in the st group compared to backgroud and a recovery period of 15–20 min. The amplitude of the systolic and diastolic wave was increased in both groups. Amplitude of incisura amplitude and rheographic index were also higher. Vascular resistance was increased in 1t group and in the 2nd group decreased by 5–10 min. Conclusions. The obtained results indicated that the reactions of peripheral hemodynamics to the influence of IPC depended from the individual values of the level of blood filling of the lower extremities. | en-US |
| dc.description | РЕЗЮМЕ. Одним із терапевтичних методів, що довів свою клінічну ефективність як перспективну альтернативу фармакологічним і хірургічним втручанням, є об’ємний пневмопресинг. Мета – вивчення реакції периферичної гемодинаміки на вплив об’ємного пневмопресингу (ОП) залежно від рівня кровонаповнення нижніх кінцівок. Матеріал і методи дослідження. У дослідженні брали участь 62 особи віком від 18 до 22 років, яким впродовж 20 хв здійснювали ОП інтенсивністю 40 мм рт. ст. приладом Recovery Pump RPX 2020 Compession (“Mego Aftek AC Ltd”, Ізраїль). Зміни параметрів периферичної гемодинаміки визначали за допомогою реовазографії, приладом ХАІ-medica standard (“ХАІ-medica”, Україна), тривалістю 15 с. Для визначення величини кровонаповнення нижніх кінцівок використали формулу, запропоновану вченими Н. Р. Палеєвим, І. М. Каєвіцером, КН=L2/Z, де КН – рівень кровонаповнення нижніх кінцівок, L – відстань між електродами струму (см), Z – рівень базового імпедансу (Ом). За значеннями цих показників визначили І групу, з відносно низьким рівнем КН, від 2,16 до 5.9, (n=30) та ІІ групу, з відносно високим КН, від 6,01 до 11,01, (n=32). Результати. Аналіз показників периферичної гемодинаміки між групами виявив, що в період відновлення А1 збільшувався в осіб ІІ групи, А2 – зменшувався. Під час відновлення спостерігалось зменшення ФПХ в І групі, а в ІІ – збільшення ІЕСС. Вплив об’ємного пневмопресингу зумовив збільшення T в І групі, порівняно з станом спокою та періодом відновлення, на 15–20 хв. Показники Ас, Ад, Аі та РІ були вищими в обох групах. ЗПОС статистично збільшувався в І групі, а в ІІ групі – знижувався на 5–10 хв. Висновки. Отримані результати вказують, що реакції периферичної гемодинаміки на вплив ОП різняться залежно від індивідуальних особливостей рівня кровонаповнення нижніх кінцівок. | uk-UA |
