SERUM CYTOKINE AND ADIPOKINE LEVELS IN PATIENTS WITH GOUT, THEIR ASSOCIATION WITH COMORBIDITY

dc.descriptionSUMMARY. Gout is a common, recurrent and potentially destructive arthritis associated with cardiometabolic-renal diseases. However, the underlying mechanisms of a high comorbidity burden are still not elucidated. The aim – to analyze circulating levels of interleukin-1 beta (IL-1β), IL-6, IL-8, IL-10, leptin (L), adiponectin (A), A/L ratio, and determine their relationship with the gout-specifically modified Rheumatic Diseases Comorbidity Index (mRDCI) in patients with gout. Material and Methods. This prospective study included 136 male patients with gout. The patients were divided into 4 subgroups based on the mRDCI values (0, 1–2, 3–4, and ≥5 points): 20 patients without comorbidities, 28 – with low сomorbidity index (CI), 62 – moderate CI, and 26 - high CI, respectively. The control group consisted of 31 practically healthy people. The serum levels of IL-1β, IL-6, IL-8, IL-10, adiponectin, and leptin were determined using an enzyme-linked immunosorbent assay. For a comprehensive assessment of dysadipokinemia we used the A/L ratio (with Adpn concentrations expressed in μg/ml and Lep levels in ng/ml). Results. The serum levels of IL-6 were exceeded by 5.16 times in patients with gout in comparison with the control group, as well as IL-8 – by 4.05 times, IL-10 – by 2.7 times, IL-1β – by 2.6 times, leptin – by 2.3 times. Meanwhile, the A/L ratio was 4.2 times lower compared to the control group and the circulating adiponectin levels – 1.7 times (for all cytokines and adipokines p<0.001). We found a persistent and significant trend towards an increase in the serum levels of IL-1β, IL-6, IL-8, and leptin, while a decrease in the levels of IL-10, adiponectin, and the A/L ratio between mRDCI subgroups in the direction from the one without comorbidities to the one with high CI (p<0.001) in patients with gout. Conclusion. An increase in serum levels of pro-inflammatory IL-1β, IL-6, IL-8, leptin, and a decrease in anti-inflammatory IL-10, adiponectin, the A/L ratio reflecting inflammatory burden in patients with higher mRDCI.en-US
dc.descriptionРЕЗЮМЕ. Подагра – це поширений рецидивний та потенційно деструктивний артрит, що асоціюється із кардіометаболічними та нирковими захворюваннями. Однак механізми, що лежать в основі значного тягаря коморбідності, досі не з’ясовані. Мета – провести аналіз циркулюючих рівнів інтерлейкіну-1 бета (ІЛ-1β), ІЛ-6, ІЛ-8, ІЛ-10, лептину (Л), адипонектину (А), співвідношення А/Л та визначити їх зв’язок із модифікованим для подагри індексом коморбідності ревматичного захворювання (анг. – the gout-specifically modified Rheumatic Diseases Comorbidity Index (mRDCI). Матеріал і методи. У проспективне дослідження включили 136 хворих з подагрою, яких поділили на 4 підгрупи згідно зі значеннями mRDCI (0, 1–2, 3–4 та ≥5 балів): 20 пацієнтів без коморбідності, 28 – з низьким, 62 – із середнім та 26 – із високим індексом коморбідності (ІК) відповідно. Контрольна група – 31 практично здорова особа. Сироватковий рівень ІЛ-1β, ІЛ-6, ІЛ-8, ІЛ-10 у пг/мл, адипонектину та лептину визначали методом імуноферментного аналізу. Для комплексної оцінки дисадипокінемії визначалось співвідношення між сироватковим вмістом адипонектину в мкг/мл та лептину в нг/мл. Результати. Сироватковий рівень ІЛ-6 у хворих з подагрою був у 5,16 раза вищим при зіставленні з групою практично здорових осіб, як і ІЛ-8 – у 4,05 раза, ІЛ-10 – у 2,7 раза, ІЛ-1β – у 2,6 раза та лептину – у 2,3 раза відповідно. З іншої сторони, співвідношення А/Л було у 4,2 раза нижчим, порівняно з контрольною групою, як і циркулюючий вміст адипонектину – у 1,7 раза (для всіх цитокінів та адипокінів, p<0,001). Нами відзначено стійку достовірну тенденцію до зростання циркулюючих рівнів ІЛ-1β, ІЛ-6, ІЛ-8, лептину, але зниження ІЛ-10, адипонектину, а також співвідношення А/Л між підгрупами за mRDCI у напрямку від без коморбідності до високого ІК (p<0,001). Висновок. Підвищення сироваткових рівнів прозапальних ІЛ-1β, ІЛ-6, ІЛ-8, лептину та зниження протизапальних ІЛ-10, адипонектину, а також співвідношення А/Л відображає запальний тягар у пацієнтів з подагрою з вищим mRDCI.uk-UA
