APPROACH IN COMPREHENSIVE REHABILITATION OF PATIENTS AFTER DAMAGED MENISCUS RESTORATION OF THE KNEE JOINT BY STITCHING THEM.

dc.descriptionSUMMARY. The results of biomechanical studies have shown that the loss of the integrity of the meniscus leads to a marked change in kinematics and load distribution in the knee joint, which is one of the prerequisites for the development of gonarthrosis. It is believed that suturing the damaged meniscus prevents early degenerative changes in the joint, especially in younger patients. The aim – to evaluate the clinical and functional results after suturing meniscus ruptures using all-inside and inside-out techniques and to determine the features of the team approach in the rehabilitation of patients in this category. Objectives of the study – to determine the theoretical and methodological principles of the team approach in the rehabilitation of this category of patients. Material and Methods. During the period from 2015 to 2021, 47 patients aged from 14 to 42 years, were performed meniscus suturing under arthroscopic control. Men accounted for 61.90 % (26), women – 38.10 % (16). The age of the subjects in men was 26.4±2.1 years, women – 22.1±3.1 years. The mean time from injury to surgery was (28.4±3.2) days. FAST-FIX ™ 360 and Arthrex 2.0 Fiber Wire Meniscus Repair Needles were used to stitch the meniscus. Results. According to the J. Lysholm rating scale, the following results were obtained: "excellent" – in 27 (64.29 %) patients, "good" – 8 (19.05 %), "satisfactory" – 5 (11.90 %) and "unsatisfactory" – in 2 (4.76 %) patients. Conclusions: The results of surgery are influenced by the type, location of meniscus damage and the time since the injury; The use of FAST-FIX ™ 360 and Arthrex 2.0 Fiber Wire Meniscus Repair Needles allows you to safely suture meniscus tears and gives good clinical results in the early observation period. The functional results of surgical interventions in suturing meniscus ruptures are influenced by the team approach in the rehabilitation of patients with this pathology, which involves close cooperation between the operating surgeon, rehabilitologist and patient.en-US

Результати біомеханічних досліджень показали, що втрата цілості меніска приводить до помітної зміни кінематики та розподілу навантажень у колінному суглобі, що є однією з передумов розвитку гонартрозу. Вважають, що зшивання ушкодженого меніска запобігає раннім дегенеративним змінам у суглобі, особливо у пацієнтів молодшого віку. Мета – оцінити клінічні та функціональні результати після зшивання розривів менісків із використанням технік «all-inside» та «inside-out» та визначити особливості командного підходу в реабілітації пацієнтів даної категорії. Визначити теоретичні та методологічні засади командного підходу в реабілітації вказаної категорії пацієнтів. Матеріал і методи. За період з 2015 по 2021 рік включно у 47 пацієнтів віком від 14 до 42 років проведено зшивання менісків колінного суглоба під артроскопічним контролем. Чоловіків було 61,90 % (26 хворих), жінок – 38,10 % (16 хворих). Вік обстежуваних чоловіків був (26,4±2,1) років, жінок – (22,1±3,1) років. Середній час від моменту отримання травми до оперативного втручання становив (28,4±3,2) дні. Для прошивання менісків застосовували системи FAST-FIX ™ 360 та Arthrex 2.0 Fiber Wire Meniscus Repair Needles. Результати. За оціночною шкалою J. Lysholm отримано такі результати: «відмінні» – у 27 (64,29 %) пацієнтів, «хороші» – у 8 (19,05 %), «задовільні» – у 5 (11,90 %) та «незадовільні» – у 2 (4,76 %) пацієнтів. Висновки. На результати оперативного втручання впливають вид, локалізація ушкодження меніска та час з моменту виникнення травми. Використання систем FAST-FIX™ 360 та Arthrex 2.0 Fiber Wire Meniscus Repair Needles дозволяє безпечно зшивати розриви менісків і дає хороші клінічні результати в ранньому періоді спостереження. На функціональні результати оперативних втручань при зшиванні розривів менісків впливає командний підхід в реабілітації пацієнтів з даною патологією, який передбачає тісну співпрацю оперуючого хірурга, реабілітолога та пацієнта.uk-UA
