dc.descriptionProper medical records are an important part of all healthcare settings. During clinical practice, the physical therapist can evaluate, diagnose, and intervene within his or her competence, so there is a need to document these actions. The article presents the experience of teaching the elective course “Medical documentation in rehabilitation institutions” for bachelors in “Physical Therapy. Occupational therapy. “ The purpose of the work is to emphasize the attention of teachers and students to the importance of gaining knowledge about the importance of medical documentation in the work of rehabilitation institutions in the modern system of professional training of specialists in physical therapy. The physical therapist should keep clear and well-organized documentation of the results of the diagnosis and evaluation and develop a plan of action. These records are entered into the patient’s medical record by the physical therapist to provide evidence of contact with the patient, to inform the clinical justification process, and to pass information to other professionals. An important task in the training of specialists in physical therapy is to study the basics of documentation that allows you to identify, register, measure and evaluate rehabilitation, as well as determine the effectiveness of rehabilitation, analyze the patient’s condition in subsequent periods of rehabilitation.en-US
dc.descriptionПравильне ведення медичної документації є важливою складовою діяльності всіх закладів охорони здоров’я. Під час клінічної практики фізичний терапевт може проводити оцінювання, діагностування та втручання в межах своєї компетенції, тому виникає необхідність у документуванні цих дій. У статті представлений досвід викладання вибіркової дисципліни «Медична документація в реабілітаційних закладах» для бакалаврів зі спеціальності «Фізична терапія. Ерготерапія». Мета статті – акцентувати увагу викладачів та студентів на важливості отримання знань про значення медичної документації в роботі реабілітаційних закладів у сучасній системі професійної підготовки фахівців з фізичної терапії. Фізичний терапевт повинен вести чітку й добре організовану документацію результатів діагностики та оцінки і розробляти план подальших дій. Ці записи вносяться до медичної карти пацієнта фізичним терапевтом для надання доказів контакту з пацієнтом, інформування процесу клінічного обґрунтування та передачі інформації іншим фахівцям. Важливим завданням при підготовці спеціалістів з фізичної терапії є вивчення основ ведення документації, що дозволяє ідентифікувати, реєструвати, вимірювати та оцінювати реабілітацію, також визначати ефективність реабілітації, аналізувати стан пацієнта в наступні періоди реабілітації.uk-UA
