THE EVOLUTION OF CYTOPATHIC HYPOXIA AND CYTOKINE PROFILE DEPENDING ON THE STAGE OF PURULENT PERITONITIS

| dc.description | A comparative analysis of the contents of cytokines and indicators cytopathic hypoxia (hemoglobin compounds pool –carboxyhemoglobin and methemoglobin) in the blood of patients with purulent peritonitis (PP) of the defi nition of their diagnosticvalue in determining the severity of peritonitis and choosing perioperative tactics. Level of IL-1, IL-2, IL-4, IL-6, TNF-α duringhospitalization in 126 patients PP was determined. It was stated that with progression stages PP (reactive and toxic) patientshad a progressive increase of proinfl ammatory cytokines (TNF-α, IL-1, IL-6 and IL-2) and anti-infl ammatory cytokine IL-4,in patients with terminal stage, sharp reduce the concentration of IL-2, IL-4, IL-6 was determined, with continued growthlevels of IL-1, TNF-α. Research of COHb and MtHb levels showed a progressive increase in the progression of PP. Defi nitionof indicators cytokine profi le and cytopathic hypoxia showed high diagnostic value in determining the severity of the PP. Highcost, duration and technical complexity make it impossible for daily use cytokine profi le indicators in clinical work, while thedetermination markers of cytopathic hypoxia (COHb, MtHb) as method of determining the severity of peritonitis is possiblefor all levels of surgical departments. | en-US |
| dc.description | Проведено порівняльний аналіз вмісту цитокінів та показників цитопатичної гіпоксії (сполук пулу гемоглобіну – кар-бокси- та метгемоглобіну) в крові у хворих з гнійним перитонітом (ГП) із визначенням їх діагностичної цінності привстановленні тяжкості перитоніту і виборі періопераційної тактики. Досліджено рівні ІЛ-1, ІЛ-2, ІЛ-4, ІЛ-6, ТНФ-α наетапі госпіталізації у 126 хворих на ГП. Констатовано, що із прогресуванням стадій ГП (реактивна і токсична) у хворихспостерігали прогресивне зростання прозапальних цитокінів (ТНФ-α, ІЛ-1, ІЛ-6 та ІЛ-2) та протизапального цитокінуІЛ-4, у хворих з термінальною стадією спостерігали різке зменшення концентрації ІЛ-2, ІЛ-4, ІЛ-6 при термінальнійстадії ГП, при продовженні зростання рівнів ІЛ-1, ТНФ-α. Дослідження рівнів COHb та MtHb показало прогресивнезбільшення обох показників при прогресуванні ГП. Визначення показників цитокінового профілю та цитопатичноїгіпоксії показало високу діагностичну цінність при встановленні тяжкості ГП. Висока вартість, тривалість, технічнаскладність унеможливлюють щоденне використання показників цитокінового профілю в клінічній роботі, тоді як виз-начення маркерів цитопатичної гіпоксії (COHb, MtHb) як скриніговий метод визначення тяжкості перитоніту є можли-вим для хірургічних стаціонарів усіх рівнів. | uk-UA |
