Abstract
Recent demographic trends demonstrate that elderly patients constitute a significant part of our society, and their health and care are issues that require special attention and expert approach. As a person ages, they undergo changes in their physiological status, which makes them increasingly vulnerable to various diseases and health problems. In this study, we explore in depth the nursing aspects of caring for elderly patients, focusing on several key aspects such as falls, diabetes mellitus, asthma and dementia, and prevention of these problems. The aim of the study was to investigate the nursing process in the care of geriatric patients, creating plans for patient-centered stages of management and nursing actions for falls, diabetes mellitus, asthma and dementia, and preventive interventions for these conditions. Older patients who were hospitalized with fall-related injuries, diabetes, asthma and dementia were the object of research in this study. Our research study has used the bibliosemantic, analytical and statistical methods, and general clinical assessments in gerontological patients. As a practical aspect, the study involved development of personalized care plans, expanding nursing competences through training in new methods and technology, and optimization of treatment outcomes with a more adjustable and patient-centered care. Additionally, the results of the study can provide a framework for further research into the different contexts of care and empowering nursing practice to improve the quality of life of elderly patients.
