dc.description.abstract | We are trying to bring attention and focus on the important role the nursing profession plays in improving patient’s experience and outcomes. Nurses are best positioned at the center of information exchanges and play critical through their routine interventions. Tasks that are often perceived as simple routine tasks, like: care planning, medication reconciliation, communications with the patient and other professionals, discharge instructions and education, are the very foundation of the successful care coordination intervention. | uk |
