Nurses are best positioned at the center of information exchanges and play critical through their routine interventions. Tasks that are often perceived as simple routine tasks, like: care planning, medication reconciliation, communications with the patient and other professionals, discharge instructions and education, are the very foundation of the successful care coordination intervention.uk
